1. Beneficiary receives Ticket and calls MAXIMUS
2. MAXIMUS gives information about VR Offices
ENs in their area.
11. MAXIMUS assigns ticket to VR and sends
confirmation letter to VR HQ-TTW Unit
3. Beneficiary meets with VR and/or ENs to
discuss goals and services.
10. MAXIMUS receives and processes the 1365.
9. Beneficiary and VR complete/sign State VR
Agency ticket assignment form (SSA-1365) and
forwards 1365 to MAXIMUS.
4. Beneficiary consults with WIPA
5. Beneficiary chooses VR
8. VR and beneficiary develop and sign IPE
6. VR confirms ticket status with
MAXIMUS
7. MAXIMUS verifies ticket assignability
