HSE ALERT
Man crushed by falling BOP - FATALITY

• Rig hands were preparing an adaptor spool for
  mounting a single BOP.
• Before the preparation was complete the forklift
  arrived at the scene carrying the BOP.
• The operator kept the load suspended
  approximately 1 metre off the ground close to the
  workers for around 15 minutes.
• Over the time, the forks slowly drooped down
  until eventually the BOP slid off the forks.
• One man was struck on the lower leg and suffered
  a deep cut to his calf.
• A second man was trapped and crushed with both
  legs under the BOP.
• The second man died later as a result of his
  injuries.
• The accident would have been avoided simply by
  lowering the load to the ground whilst waiting
  for the preparation to be completed, or by
  waiting a safe distance from the work.
• Poor planning (lifting the BOP instead of
  adaptor) created a higher risk job.
