• Audit patients discharge documentation at time of
  discharge.
• Audit patients views and satisfaction post
  discharge.

Establish Multidisciplinary Discharge Work
GroupAims

• To unify Discharge Planning Documentation
  throughout North Glasgow University Hospitals
  Division.
• To provide all Wards in North Glasgow University
  Hospitals Division with clear unified Discharge
  information on Intranet and in folders at ward
  level.

Joint Education Sessions

• Provide sessions for all NGUHD staff and all
  other professional disciplines related to
  Discharge Planning.
• Use audit results to establish gaps in knowledge.
• Education will improve gaps and provide a
  uniformed approach to Discharge Planning.

• Discharge Lounges.

• Improve joint collection and verification of
  Delayed Discharges.
• Develop and expand monthly Discharge meetings.
