• INCIDENT
• IP was handling chemical bags from the edge of
  the mixing skid and passing them to another
  Roustabout who was mixing chemicals into the
  hopper. They both were working on the Mixing skid
  without installing the handrails gates although
  they were was 2 meters above ground level. The IP
  lost his balance and fell down to the ground and
  received a hairline fracture on his right wrist.
• CAUSES
• Handrail Gates not installed and closed
• Risk assessment not done properly during TBT
• IP and all crew failed to recognize the hazard
  related to working at height without handrails
• LEARNINGS
• Identify and discuss all hazards at the workplace
  prior to commencing any job, in a structured
  Toolbox Talk.
• Never Remove Handrails
• General JSAs to be updated to suit Rig and
  operations
