LSN Lacrosse Checklist

• I have a US Lacrosse Number.
• Visit www.uslacrosse.org and become a High School
  member. (Cost is 35.)
• Register at http//www.leessummitlacrosse.com/lsn
  /.
• I have a lax stick. (Must meet NCAA standards)
• Note Sticks are different for men womens
  games.
• I have cleats. (soccer or football cleats will
  do)
• I have a valid physical on file at the school.
• I and my parents have signed the waiver form from
  on-line registration listed above.
• I have been fitted for my uniform needed this
  season and paid LSN Lacrosse
• Safety Equipment Needed
• Boys - Helmet, Shoulder Pads, Elbow Pads, Gloves,
  Athletic Cup, Mouthpiece
• Girls - Goggles, Mouthpiece. (Girls LAX gloves
  are optional)
• If you have checked them all, then you are ready!
