- Whenever healthcare is provided, medical records
must be created, reviewed and stored. - These records can include any or all of the
following - 1. Lab results- blood tests, urine tests etc.

- 4. Clinical information (nursing notes,
physical therapy notes, and many others) - 5. A History and Physical Exam
