document until your teachers instructs you to do
so. It must be filled out following the very
specific instructions that follow - Teachers Please read the information on the
following slides EXACTLY.
3Answer Document Section 1
- Today you will be filling out your identifying
information on the PSAT/NMSQT answer sheet. - Use only a No.2 pencil. Raise your hand if you
do not have one. Begin by reading the directions
in the box above Section 1. Then enter your
name, LAST NAME FIRST, and fill in the
appropriate ovals carefully. Make sure each mark
is dark and completely fills in the oval - (Proceed NOW to the next slide for a picture of
the answer document)

5Answer Document Sections 2, 3, 4
- In Sections 2, 3, and 4, fill in the appropriate
ovals. - When completing Section 3, Date of Birth,
select the correct month, day, and year of your
birth. Fill in the zero oval in the left column
if your birthday is prior to the tenth day of the

- Enter your social security number in Section 5.
If you do not have a social security number,
cannot remember it, or do not wish to give it,
leave this section blank. Do not enter any

14Answer Document Sections 7-12
- Sections 7-12 are included to help the College
Board help you. Some of the information you
provide will help ensure that tests are fair for
all groups. Some of it can help colleges,
universities, and nonprofit educational
opportunity organizations, and some scholarship
programs provide you with relevant information
for college planning. Your answers to these
questions will not affect your participation in
the competitions conducted by National Merit
Scholarship Corporation.

- Complete Sections 7, 8, 9a, and 9b. Mark only

- Section 10 asks if you wish to participate in the
College Boards Student Search Service, a free
service that provides names of students to
certified educational organizations. You must
fill in the Yes oval in Section 10a to let
colleges, universities, scholarship programs,
educational opportunity organizations, and the
College Board know that you are interested in
receiving information about the educational and
financial aid opportunities they offer.
18Answer Document Sections 7-12
- Colleges, universities, and scholarship programs
that request it will receive your name, address,
and some other information you provide on this
answer sheet. They will NOT receive your actual

colleges and educational organizations.
20Answer Document Sections 7-12
- If you marked Yes and would like to receive
information via e-mail, print your e-mail address
in the spaces provided in section 10b. Colleges
and scholarship programs may prefer to contact
you viae-mail. Indicate if this is your e-mail
address or a parent/guardians address. If you
do not have an email address, you can still
receive information by regular mail. Leave

22Answer Document Sections 13 14
- Turn your answer sheet over to page 4. Sections
13 and 14 relate to scholarship programs
conducted by National Merit Scholarship
Corporation. Complete all parts of Section 13
if you omit a part, NMSC cannot determine if you
meet requirements to enter its scholarship
programs.
23Answer Document Sections 13 14
- In section 13a, indicate whether you are enrolled
as a full-time high school student. - In Section 13b, fill in the oval next to the year
in which you will complete or leave high school
and enroll full time in college. For example,
11th graders who are planning to complete high
school and enter college in 2010 should code
2010. Tenth graders should code the year in
which they will complete high school, which in

25Answer Document Sections 13 14
- In Section 13c., fill in the oval next to the
number of years you expect to spend in grades 9
through 12. If you spend one year in each grade,
you will complete high school in 4 years. - In Section 13d., if you are a United States
citizen, fill in the Yes oval. If you are not
a U.S. citizen, fill in the No oval that
applies to you. - (Proceed to the next slide for a picture of the

- If you are a Black American and wish to enter the
National Achievement Program as well as the
National Merit Program, fill in the Section 14
oval in addition to completing Sections 13a. to

28You may only fill in Section 14 if you are an
African-American. This is optional.
29Answer Document Sections 15-21
- Now carefully read directions in the shaded box
above Section 15. To receive mail from
scholarship programs, colleges, and universities,
and to be able to access My College QuickStart
online, you must fill in the address ovals
correctly. Include your apartment number in 15,
if you have one. If necessary, use the address
abbreviations in the lower right of your answer
sheet.
30Answer Document Sections 15-21
- Indicate a space in your address by leaving a
blank box and filling in the corresponding
diamond. If your address has a slash mark or a
hyphen, fill in the corresponding slash mark or
hyphen oval. Be sure to complete your 5-digit
zip code in Section 18. - Leave Sections 20 and 21 blank at this time.

your mail. If you get mail at aP.O. Box, use
that address.

- Students Tomorrow morning, Breakfast Break is
before school it ends at the first bell at 824.
You MUST be in your Advisory class by 830. - Remember to bring a calculator for the math
portion of the test! You may NOT use the
calculator in your cell phone, or any other
calculator that has an alphabet keyboard. You
may not share a calculator, so bring your own.
