  • The drug treatment of heart failure, once simple,
    has become complex. Apart from a loop diuretic
    and digoxin, most patients should now be
    receiving an angiotensin-converting enzyme
    inhibitor (or angiotensin II receptor blocker), a
    beta-blocker and spironolactone.

Benefits of angiotensin converting enzyme (ACE)
blockade
  • Heart Outcomes Prevention Evaluation (HOPE) study
    conclusively demonstrated that ramipril, an
    angiotensin-converting enzyme (ACE) inhibitor,
    reduces the risk of cardiovascular death.
  • Patients Treated with Ramipril and Vitamin E
    (SECURE) substudy demonstrated that ramipril also
    reduced atherosclerosis. These results suggest
    that the renin-angiotensin system (RAS) has a
    more important role in the development and
    progression of atherosclerosis than previously
    believed.
