2As You Receive FCAT SSS TEST MATERIALS From The
Test Administrators, Do The Following
___1. Verify that all distributed secure
materials have been returned. Notify the
district coordinator immediately if any secure
materials are missing and complete the necessary
investigation. ___ 2. Make copies of the
records of required administration information
collected by test administrators and file the
copies. (Page 24 of the manual) ___ 3. Verify
that your SSS FCAT Security Log has been
completed correctly. Make a copy for your
files. ___ 4. Remove stray SSS math reference
sheets or science SSS reference sheets/periodic
tables from the test books and answer
documents. ___ 5. Verify that each student has
completed the required information (Student Name,
District/School) in the upper right corner of
the student grid sheet, even if a label has been
affixed in the lower left corner.
3- ___ 6. Separate the SSS test materials into
stacks as follows -

tested. Page 229 of the manual - Verify that you have included all documents from

DOCUMENT COUNT FORM. - The SSS Count Form is scannable and must not be
photocopied. -
- Place each completed SSS Count Form on top of the
first stack of TO BE SCORED answer documents.

SUMMARY as instructed below - Complete only one SSS ANSWER DOCUMENT RETURN
SUMMARY for students enrolled in your school.
There are separate SSS ANSWER DOCUMENT RETURN
SUMMARY Forms for Home Education, McKay
Scholarship Program and CTC Scholarship
Programs. - Transfer the information from each SSS DOCUMENT
COUNT FORM to the SSS ANSWER DOCUMENT RETURN
SUMMARY, making sure that the Number of TO BE
SCORED Documents Returned for each grade level
and document type is correct. - Verify that the number of documents returned and
the number of documents indicated on the SSS
DOCUMENT COUNT FORMS match the entries on the
SSS ANSWER DOCUMENT RETURN SUMMARY. - Maintain a copy of the completed document for
your records.

DOCUMENT RETURN SUMMARY FORMS which have been
provided for each school. - Complete the Number of Answer Documents

You may use more than one SSS Paper Band per

Follow these steps for each paper band you use - Grid the appropriate grade level and document
type. - Write the school name and district/school
number at the bottom of the SSS Paper Band. - Count the number of TO BE SCORED documents that
will be banded in each SSS paper band. Write
that number under Number of To Be Scored
Documents Contained in This SSS Paper Band. - Under Number of Banded Stacks per Document
Count Form, number the SSS Paper Bands 1 of n,2
of n, etc., where n is the schools total number
of banded stacks per SSS Document Count Form. - Place an SSS paper band around each stack of TO
BE SCORED answer documents. The SSS Document
Count Form must be under the paper band on top
of the stack labeled 1 of n. - Seal the SSS Paper Band and be sure that it
securely holds the documents. Do not use
staples, paper clips, or tape.

enrolled in Public Schools will be processed by
using the number for the school in which they are
enrolled. Scores for these students will be
returned to your school. TO BE SCORED materials
for McKay Scholarship students enrolled in your
school should be returned with your schools TO
BE SCORED materials.

Calibration Materials on top of the documents in
the first box of TO BE SCORED calibration
