Patient

• Young male patient returned from 2 years of
  service in the Peace Corps in Brazil
• Presents at his primary physician with fever,
  chest pain, breathlessness, and a distended
  abdomen
• Was in car accident and received a blood
  transfusion in a local Rio de Janeiro hospital
• Chest X-ray shows both hepatosplenomegaly and
  heart enlargement and an EKG shows a systolic
  murmur
• 2 years later patient dies from heart damage
  caused by hemoflagellate trypanosomal protozoa
  infecting him

Differential Diagnosis

• Trypanosoma brucei gambiense
• Tryponosoma cruzi
• Cardiomyopathy
• Acute Respiratory Distress Syndrome (ARDS)
• Bronchiectasis

• CBC (Complete Blood Count)
• Blood Specimen (Blood Smear)
• Indirect Aggutination
• ELISA (Enzyme-linked Immunosorbent Assy)
• CSF (Cerebral Spinal Fluid)

• Patient is diagnosed with Trypanosoma brucei
  gambiense, also known as African Sleeping
  Sickness. Blood smear reveals the Trypanosoma in
  blood.

Mode of Transmission

• Trypanosoma brucei gambiense can be contracted
  by the tsetse fly found in Africa. It can also be
  contracted by a blood transfusion.

How Transmition Occurred

• Patient contracted disease due to the blood
  transfusion he had in Rio de Janeiro.

Prevention

• Contraction could have been prevented if
  hospital would have screened the donors blood
  for any infections.
