Personal Health
  • Students will
  • Engage, from Grade 10 to Grade 12, in at least 80
    hours of moderate to vigorous physical activity
    in addition to P.E. 10.
  • Develop a long-term personal healthy living plan
    appropriate to their lifestyle that describes
  • a. sound nutritional habits
  • b. regular exercise routines
  • c. emotional health (stress) management
  • d. positive healthy choices

  • To meet this requirement students must
  • Document and describe the 80 hours you have
    completed and have them verified with either
    supporting documents or an adults signature.
  • Develop a long-term personal healthy living plan
    that includes
  • a. sound nutritional habits
  • b. regular exercise routines
  • c. emotional health management
  • d. positive health choices

Community Connections
  • Students will
  • Participate in at least 30 hours of work
    experience and/or community service.
  • Describe the duties performed, the connections
    between the experience and their employability
    and life skills, and the benefit to the community
    and to the student.
  • (a.k.a. reflection!)

  • To meet this requirement students must
  • Complete Confirmation of Volunteer or Work
    Experience Form and have it signed by boss or
    supervisor.
  • Describe the experiences and how they connect to
    developing skills and abilities.
  • Show the connection to their future plans.

Career and Life
  • Students will
  • Develop a plan that indicates they are prepared
    to successfully transition from secondary school,
    which includes
  • -identification of career, life and learning
    goals
  • -evidence of transition activities that are
    relevant to their career, life and learning
    goals.
  • -evidence of achievements that support your
    career, life and learning goals.
  • -a comprehensive budget to show that you have
    considered how to finance your plan.

  • To meet this requirement students must
  • Complete a Career and Life Transition Planning
    document that will include evidence collected by
    the student.
  • Present their Transition Plan to a staff member
    (administrator, counsellor, CP teacher) during a
    Transition Interview.

What will my mark be?
  • Students must complete the minimum requirements
    in order to earn credit for Graduation
    Transitions. Upon successful attainment of those
    requirements, a Requirement Met (RM) mark will
    be assigned, and students will earn (4) credits
    towards graduation.

  • Ensure that you schedule an appointment with a
    counsellor.
  • The purpose of this meeting is to ensure all your
    grad requirements are in order and at this time
    you can present your documents and reflections to
    complete your Grad Transition Plan.
