MATERIALS NEEDED

• Collection kit.
• Requisition form.

MATERIALS NEEDED

• Scissors or thinning shears.
• Comb
• Hair clip
• Rubbing alcohol pad.

INSIDE THE KIT

• Envelope
• Envelope seal
• Specimen bag
• Foil wrapper with root end notation

Step 1 Identify the Donor

• Identify the donor by a valid photo ID.
• Enter the Donors Name on the Requisition form.
• Enter a unique identification number for the
  donor on the Requisition form (Social Security
  Number, Drivers License or Employee Number, etc).

Step 2 Prepare the Site

• Wipe the scissors or shears with the alcohol pad.
• Part the hair using the comb and hair clip.

Step 3 Cut the Hair

• Isolate approximately 200 strands of hair.
• 200 strands is as big around as a 2 pencil.
• Cut the hair as close to the scalp as possible.

• Refer to the visual aide to determine if an
  adequate sample has been harvested.

• Alternatively, for thinning hair, use the
  thinning shears and collect clippings in the
  envelope or a sheet of paper.

Step 4 Align the Hair

• Align the hair specimen in the foil wrapper.
• Make sure that the root end is toward the Root
  End marking on the foil.

Step 5 Secure the Hair

• Fold the hair specimen in the foil wrapper, much
  like a stick of chewing gum.

Step 6 The Envelope

• Place the wrapped hair specimen in the envelope.
• Seal the envelope with the supplied tamper
  evident seal.

Step 7 The Envelope

• Mark the donor ID
• City/Town of collection
• Donors initials
• Collector Signature and date/time

Step 8 The Barcode Sticker

• Remove a barcode sticker from the bottom right of
  the requisition form.
• Place the sticker on the top right corner of the
  envelope flap.

Step 9 Requisition form

• Mark the desired panel
• Mark the collection site, date and time
• Donor signs the donor statement.
• Collector signs collector statement

Step 10 The Specimen Bag

• Place the sealed hair specimen in the front
  pocket of the specimen bag.
• Fold the requisition form twice and place it in
  the back pocket.

Step 11 Shipment

• The specimen is now ready for shipment to the
  laboratory.
