• To have Aging and Disability Resource Centers in
  every community serving as highly visible and
  trusted places where people of all incomes and
  ages can turn for information on the full range
  of long term support options and a single point
  of entry for access to public long term support
  programs and benefits.

• Must serve individuals 60 and at least one
  target population of people with disabilities
• Serve individuals at all income levels
• Assist individuals planning for future LTC needs
• Establish formal linkages w/ critical pathways to
  LTC
• Open a Resource Center meeting all the
  requirements in at least one community

Evidence-Based Prevention Program

• 2003 AoA launched a new discretionary grants
  program
• Grants will demonstrate the efficacy of
  delivering evidence-based prevention programs
  through community-based aging service providers.
• Health promotion and disease prevention is a
  major AoA priority.

• Public/private partnership with CDC, NIA, AHRQ,
  RWJ Foundation, Hartford Foundation, and the
  Horizon Foundation
• 12 grants awarded in the following areas
• - physical activity - nutrition

• All projects have succeeded in implementing
  interventions.
