University of North Carolina, and several Florida

- Minimal premium increases for employees
- Individual coverage increasing from 48.68 to
50.00 per month - Family coverage increasing from 175.14 to
180.00 per month - Represents less than 3 increase in premiums to
employees - UFs employer contribution is increasing by over

- United Healthcare is being added as a new HMO
carrier - 15 counties will be gaining an HMO option where
one does not currently exist - 25 counties, including Alachua, will be receiving
a new HMO option - JMH Health Plan will no longer be contracted to
offer health care services - All HMOs will offer mail order prescription drug
benefit with similar co-payments as the state PPO

PPO/HMO
- New plan design with lower premiums and higher
deductibles - Monthly premiums are 15.00 for individual and
64.30 for family - Deductibles are 1,250 for individuals and 2,500
for family - Current HMO plan design has no deductible and PPO
design has in-network deductibles of 250 for
individuals and 500 for family - Under this plan, employee assumes greater
responsibility when health care is provided

PPO/HMO
- Employees will have the choice of a PPO option
through BlueCross BlueShield or an HMO option
through AvMed, Florida Health Care, or United
Healthcare - Employees selecting this option will be eligible
to participate in a Health Savings Account (HSA) - UF will contribute 41.66 per month for
individual and 83.33 per month for family
coverage to the HSA

PPO/HMO
- Employees may elect to contribute additional
amounts to HSA on a pre-tax basis - HSAs earn interest and function as tax-free
savings accounts - Funds can be used to cover increased employee
costs for healthcare services - Funds remaining in account at year end roll over
to future years - HSAs are portable

Reimbursement Accounts
- Limits for the MRA will increase to 5,000
- Employees with HSAs are not allowed to enroll in
an MRA - New Limited Purpose MRA is being added for those
with Health Savings Accounts - Allowable expenses under Limited Purpose MRA are
dental, vision, and preventative services - Dependent Care Reimbursement Account limit
remains at 5,000 for 2006

Term Disability
- Special Open Enrollment period being offered this
year - Open Enrollment is not offered on an annual basis
- Employees will have choice between either a 30
day elimination period or a 90 day elimination
period - Premiums for 90 day are 0.52 per 100 of covered
salary 30 day elimination period costs
additional 0.33 per 100 of covered salary

- Changes to state sponsored benefits may be made
online via the People First website at
https//peoplefirst.myflorida.com or via IVR by
calling 1-866-663-4735 - Changes to UF sponsored benefits (Term Life, Long
Term Disability) may be made by completing
appropriate forms available on the HR website at
www.hr.ufl.edu and returning the forms to the
nearest HR office on campus

Life Insurance
- Open Enrollment for Term Life moving to Fall each
year - A second Open Enrollment period will be conducted
in 2005 - Premiums adjusted as of Jan 1, 2006 due to age
recalculation - Can enroll/increase coverage by additional
10,000 of employee life and 5,000 of spouse
and/or child life without evidence of
insurability even if changes were made earlier
this year

- Departments only complete an online form
- Data entry into PeopleSoft for HR transactions
would be completed by a core set of users within
HRS. - This project has entered the design phase.
23Tuition Exchange Scholarship Program
24Tuition Exchange Program
- Provides an opportunity for dependent children of
full-time UF employees to attend a participating
Tuition Exchange college of university at a
significant tuition reduction - TEP is a scholarship opportunity rather than a
fringe benefit.
25Tuition Exchange Program
- The annual cost to the University employee to
participate in the TEP is 5,000. - Please visit the TEP website at
http//www.aa.ufl.edu/aa/provost/tep/index.htm - The application deadline is October 31st for the

530 - Benefits Fair at the Reitz Union on Oct 18 and
Oct 19 from 900am to 400 pm - Employee information sessions on the new health
plan options available each hour at Benefits Fair - If no elections made during Open Enrollment, 2005
elections will carry over to 2006
