• AIM
• To redesign the system of care in order to
  provide improved care to our patients with
  DEPRESSION. We will accomplish this by making
  changes in the following areas
• Rural Medical Services, Inc. will redesign
  the clinical practice that we are currently
  operating under in order to improve the quality
  of care to our patients who are diagnosed with
  DEPRESSION. This will be accomplished through
  the implementation of the Chronic Care Model of
  care. We will increase patient awareness of
  depression as well as treatment options, both
  short and long term as needed.

Population of Focus

• Up to 100 Patients
• Patients that are currently under the care of Dr.
  Edward Capparelli at the Grassy Fork and
  Parrottsville Centers
• Patients are selected based on diagnosis of
  DEPRESSION and their need for evaluation with
  continued support

Registry

• Referrals are made to our MSW by either the LPN
  or MD
• MSW will input data into the PECS System
• Currently there are 87 patients in our registry
• Contacts will be made in conjunction with medical
  appointments with Dr. Capparelli or based on
  severity of need.
