- Check your online roster each week to make sure

- Existing guidelines and procedures must be
followed when an instructor seeks to issue an I
grade - An Incomplete (I) Grade Form/Agreement for
Completion of Coursework form must be completed
by the instructor and signed by an Associate Dean

- How Do I Process Grade Changes For An Entire
Class? - Print your online Class Roster
- Write grades legibly next to each student
- Sign the bottom of the roster
- Have the appropriate Associate Dean sign the
roster - Submit roster to your Campus Registrars Office

individual student? - Complete the Instructor Change of Grade Form (PDF
File) - Sign the form
- Have the appropriate Associate Dean sign the form

may not be delivered by students. Sorry,
faxes/copies are not accepted.

- Absence
- If you are unable to attend a class, make sure
you notify the appropriate Associate Dean as soon
as possible to ensure that a cancellation notice
will be posted on your classroom door in time - If you must miss a class, please arrange for a
substitute or provide the class with an
appropriate alternative assignment

- Book orders are given to the chairs

- All faculty members should get a Panther Card (ID
Card) at the bookstore - Email accounts are created for all faculty

- Students evaluate one course per semester for
adjunct faculty and two courses for full-time
faculty - Student evaluation forms will be in your mailbox
mid-term - Results are distributed to faculty and associate
deans

- Adjunct faculty will be observed by their
department chair at least once per term during
their first and second terms teaching - After three terms, adjunct faculty are observed
once per academic year

- Important Paperwork
- Submit your course syllabus to the Office of
Academic Affairs during the first week of class - Make sure all final grades are submitted on time,
online, and in hard copy form on the official
grade sheet - If the grades are not electronically posted on
time, Panthernet will automatically assign an L
grade and this puts students academic records on
hold

- Key Request Procedures
- A key request form must be obtained from Academic
Affairs - It is the responsibility of the instructor or
department chairperson to contact the respective
administrative assistants to have the form
prepared - After the key request form is completed, it must
be approved by the Associate Dean, Dean, or
Provost - The form is then submitted to Facilities for
processing. Once the key(s) is ready, the
instructor will be notified to go to Facilities
in order to sign the form and pick up the keys in
person

- In the event the requestor cannot pick up the key
during Facilities business hours 730 am-400 pm,
a Security Officer will deliver the key to the
instructor at the classroom. (a notation must be
made on the key request showing the date, the
day, time and classroom the instructor will be
in) - Key requests to the multimedia desks are not
approved by Academic Affairs. Users must be
trained by the Media Services personnel and after
the training, Media will send the approved
request to Facilities

- Key Return Procedures
- It is important that all keys are returned on the
last day of the class in each term - Keys are only to be returned to the Facilities
office - Instructors who have evening or Saturday classes
can put keys in an envelop, write their name and
telephone number on the outside of the envelop,
and drop the key in the return key box (24/7) on
the southwest corner of the Facilities building
