Developer Pane
  • The Developer pane is a helpful little area of
    Microsoft Word
  • To enable the Developer pane click on the Office
    icon on the top left
  • Then click on Word Options
  • Under the Popular tab make sure Show Developer
    tab in the Ribbon is checked.

Creating the template
  • Create a new Word Document Template
  • Click the Microsoft Office Button on the top left
  • Under Templates click My templates
  • Double click Normal and then click Template or
    Document under Create New
  • Click the Microsoft Office Button and then Save
    As
  • Save it as Information_Form

Basic Form Structure
  • Enter in bold 16 point font the following text
  • Please complete all requested information.
  • Center the text
  • Enter in bold 11 point font the following text in
    separate lines
  • First Name
  • Last Name
  • Date
  • Left justify the text

Inserting a Text Fill in Field
  • On the Developer tab in the Controls group click
    on Legacy Tools
  • Click on Text form Field
  • In the Controls group click Design Mode and then
    Properties
  • Set the Maximum Length to 20 characters
  • Change Text format to First capital
  • Do this for both First Name and Last Name

Adding the date picker
  • Click next to the Date text area
  • On the Developer tab in the Controls group click
    on Date Picker
  • Uncheck Design mode
  • Save your work

Adding a Pull Down Menu
  • Add in the text Class Rank
  • Bold the text
  • On the Developer tab in the Controls group click
    on Legacy Tools
  • Click on Drop Down Form Field
  • Select the content control, and then on the
    Developer tab, in the Controls group, click
    Properties.
  • To create a list of choices, under Drop-Down List
    Properties, click Add.
  • Add in the following
  • Freshman
  • Sophomore
  • Junior
  • Senior

Adding in a check box
  • Add in the text Check if Instate in 11 point
    font and bold
  • On the Developer tab, in the Controls group,
    click Legacy Tools.
  • Click Check Box Form Field.

Add in a Calculated Field
  • Enter the following text in bold and 11 point
    font
  • Quality Points
  • Credit Hours
  • GPA
  • Make Quality Points and Credit Hours both Text
    Form Fields
  • For Quality Points and Credit Hours change the
    properties so they are Number Types with Number
    Format 0 and give them the bookmarks QP and CH
    respectively. Make sure the box Calculate On
    Exit is checked.

Doing the calculation
  • Make GPA a Text Form Field
  • Change the bookmark to GPA
  • Change the Type to Calculation
  • Enter QP/CH for the Expression
  • Change the Number Format to 0.00
  • Check the box for Calculate on Exit

Preparing the form for use
  • Without completing the following, the form cannot
    be used!
  • On the Developer tab, in the Protect group, click
    Protect Document, and then click Restrict
    Formatting and Editing.
  • In the Protect Document task pane, under Editing
    restrictions, select the Allow only this type of
    editing in the document check box. In the list of
    editing restrictions, select Filling in forms.
    Under Start enforcement, click Yes, Start
    Enforcing Protection.
  • Click OK

Testing the form
  • Save your work first
  • Close out the form
  • Open it up and enter in your information
