  • Belt
  • Buckle and tip polished (or purchase anodized)
  • No loose threads
  • Lint-free
  • Edge of tab of the belt must meet the edge of the
    belt buckle.
  • Must match uniform material.

  • Device should be parallel to leading edge of
    collar.
  • PHS Corps Device
  • Worn on left collar, pointed in same manner as
    rank device
  • Stem will be down

  • Short-sleeve khaki shirt
  • PHS Rank device worn on right collar, centered 1
    from the lower and front edges of the collar.
  • Position the vertical axis of the insignia along
    an imaginary line bisecting the angle of the
    collar point.
  • PHS Corps device Worn on left collar, centered
    as above.
  • Rank on right

Insignia Placement
  • Shoulder straps on windbreaker and raincoat
  • Large metal insignia used for jackets
  • Place rank so that outer edge is 3/4 from
    squared end of strap.
  • ENS, LTJG, LT devices are worn parallel to the
    shoulder seams
  • LCDR and CDR have the oak leaf stem 3/4 from the
    end of strap
  • CAPT will have the clutched arrows of the eagle
    3/4 from the end of strap and eagles head
    facing forward.

Insignia Placement
  • Garrison Cap (both blue and khaki)
  • Miniature (collar) rank device is positioned to
    the right 1.25 from lower edge and 1.25 from
    the forward crease.
  • Miniature (collar) PHS device is positioned on
    the left in the same manner as above.

Shoulder Boards
  • Worn with the grade closest to the shoulder.
  • USPHS insignia must have anchor pointing forward
    and not dragging backwards.
  • Hard Boards
  • Worn as above on summer white, summer blue shirt,
    and coats such as the bridge and P-coats.
  • Soft Boards
  • Worn with white long-sleeve strap shirt and Army
    V-neck sweater.

Combination Cover
  • PHS components consisting of maroon-striped chin
    strap, PHS buttons, and PHS cap device.
  • Worn flat on head, parallel to the deck.
  • Chin strap flush to brim
  • PHS buttons positioned upright
  • Clean white/khaki cover pulled snug across top.
  • PHS device centered.

Name tag
  • Worn on the right breast, 1/4 above the right
    pocket on all general purpose uniforms
  • On the Service Dress Blue, where there is no
    right pocket, place the nametag in a position
    that corresponds (mirror) to the ribbons on the
    left pocket. May need a ruler to assist.

Ribbons
  • Worn on the left breast 1/4 above the left
    pocket on all general purpose uniforms.
  • Not worn on working uniforms
  • Ensure that the ribbons are worn in proper order
    of precedence according to uniform manual.
  • Highest position of honor is inside-left of all
    other ribbons.

  • COA, AMSUS, and ROA ribbons may not be worn
    unless you have an active member (paying dues)
    and have earned at least one other ribbon.
  • Duplicate awards gold vs. bronze
  • Non-symmetrical ribbons have a specification for
    wearing (i.e.. ROA)

  • Jewelry
  • Bracelets/watch- only ONE may be worn which is
    conservative in taste.
  • Rings- only ONE may be worn on each hand other
    than the wedding band.
  • Necklace- may not be visible
  • Earrings- Uniform ONLY- small 6mm brushed gold
    ball with all GP uniforms. Small 6mm pearls only
    for formal attire.

  • Shoes
  • Heels Plain black leather, plain design, closed
    heel and toe. Heel may range from 5/8-2 5/8
    and no wider than 1 3/4 at base (no wedge type
    heel).
  • White shoes must be worn with Summer White and
    Dress White.

  • Cosmetics
  • OK if conservative and in good taste, no bright
    reds, oranges and purples.
  • Clear nail polish or shade that complements skin
    color.
  • Nails should not extend 1/4 beyond fingertips.
  • Hair
  • Neatly arranged, conservative appearance
  • None showing under front brim of cover.
  • Two barrettes are permitted if similar to color
    of hair, single french braid or two side braids
    permitted.
  • No pony tails or multiple braids
  • All hair devices should be inconspicuous and
    blend with hair color.

  • Watch and Bracelet, only one of each may be worn.
  • One ring on each hand allowed.
  • Earrings not allowed under any circumstances.
  • Necklace Must not be visible.

  • Scalp- Taper cut, neatly trimmed, no longer than
    4 on top, off the ears, may not extend below
    eyebrows, off the collar.
  • Facial-
  • Sideburns- must not extend below the earlobe,
    even-width ending in a clean horizontal line,
    neck clean-shaven
  • Mustache- should not extend below the mouth or
    outward more than 1/4. No portion below upper
    lip

Common Uniform Errors
  • Wearing wrong size Rank Insignia
  • Full size- jackets and raincoat
  • Small size- collar of khaki, winter blue shirt
    and garrison cap.
  • Lack of tie with long-sleeve shirt
  • Wearing US Navy cap devices
  • PHS does not use the all-gold chin strap or eagle
    buttons
  • Dragging anchors
  • Shoulder boards and sleeve insignia must have
    bottom of anchor pointing in a forward direction.

Common Uniform Errors
  • Coat unzipped/unbuttoned
  • 1- There IS a uniform overcoat!
  • Coat must be worn zipped or buttoned 3/4 of the
    way up or taken off.
  • SDB jacket bottom button may be un-buttoned when
    seated
  • Also an exception is made for women in maternity
    uniform.
  • Mismatch of uniform materials
  • Shirt, belt, and trousers must all be the same
    material (CNT, poly wool blend, or cotton)

Common Uniform Errors
  • Collar insignia position
  • Follow instructions on centering with the various
    uniforms (khaki is different than winter blue)
  • Gig Line
  • The shirt border, trouser zipper line, and curved
    buckle edge must fall in a straight line.
  • Gloves, handbags, and umbrellas
  • Must all be black and fit uniform specifications
    (handbag).

Common Uniform Errors
  • Shoes and Socks
  • Must match. Black with black. Brown with brown.
  • Hosiery
  • Must be as close to the natural skin color as
    possible. No black hose with SDB etc.
  • Hair too long
  • Mens length should not be more than 4 on top.
    Womens must not touch below bottom edge of
    collar.
