• To initiate a management plan for nutrition in
  HRC through use of the Rapid Screen tool
• 3 units of Department of Geriatric and
  Rehabilitation Medicine
• October 1, 2004 for 12 months

• Step 1 Screening all patients
• Admitting medical resident staff will include a

• Screen evaluation of nutrition risk as part of
• admission process
• measure height and weight for BMI
• seek estimate of weight loss
• (little / some / lots)
• Results at risk or not at risk of malnutrition

• Step 2 Streaming at-risk patients
• Nursing evaluation as part of initial nursing
• assessment using the Mini Nutritional Assessment
• tool
• Questions 1-6
• Questions 7-18 (ie remainder) if identified
• Results normal, or at risk, or already
  malnourished
