PURPOSE of the SESSION
  • To discuss the solution to how to modernise the
    administrative homecare functions at the same
    time as modernising the service.
  • To give participants an overview of how the
    program changes and the system changes produce an
    end to end solution for a modernised home care
    service.

  • Link with the Vision and the Prevention Strategy
    for LBH
  • Support from Lead Member, council, senior
    leadership and service managers for both change
    and modernisation.
  • Follows a full scale consultation on the
    modernisation of home care.

Project

OUTCOMES 1
  • A reablement home care service where service
    users are provided short term assistance to
    regain their independence and to ensure
    proscribed outcomes are met.
  • An outcome focussed reablement service that
    outlines the expected results for each service
    user, monitors their progress toward those
    results and records all activities
    electronically.

OUTCOMES 2
  • An outcome oriented commissioning process that
    supports assessment and care in reaching
    specified objectives and ensures electronic
    monitoring of all activities.
  • An electronic monitoring system that automates,
    documents and makes payments based on electronic
    monitoring data, reconciliation and
    documentation.

Key Decisions
  • Use and modification of SWIFT to record expected
    outcomes and to initiate service provision.
  • CM2000 to provide electronic monitoring of home
    care and to process work completed for payment
    directly via TDM / SWIFT
  • The use of RBS to reconcile transactions , to
    prepare electronic invoices and to pay through
    visa.
  • Ensuring direct feeds into Oracle,payroll and all
    other necessary council systems.

  • End to end solution with full audit trail
  • Budget allocation and monitoring built into SWIFT
    at the front end
  • CM2000 provides rostering, care monitoring and
    reconciliation of commissioned versus actual
    hours.
  • TDM provides for electronic payment
  • All activities entered on to FIS.
