Major Accomplishments

• Development of a cohesive multidisciplinary,
  multi-agency assertive community treatment (ACT)
  team
• Thirty four (34) participants housed, two (2)
  selecting apartments
• Refinement of funded partner relationships
• Utilization of specific funded partner expertise
  and resources
• Staff and community training featuring Dr. Sam
  Tsemberis and Dr. Robert Paulson on the evidence
  based ACT model, and housing first
• Ongoing consultation and support by Pathways to
  Housing
• Establishment of relationships with a broad array
  of community providers
