• academic programs (NOT departments), for example,
  special education , social work
• administrative areas (NOT divisions), for
  example, career services, campus police
• Changes of intended educational outcomes or
  administrative objectives
• Modification to means of assessment

• Looks for actions completed-- past tense verbs

• Provide a consistent format for us and outside
  reviewers.
