- Several State sponsored plan designs remain
unchanged for 2007 - Health PPO, HMO and Health Investor options
- Health premiums for full-time employee remain
unchanged - Dental
- Vision
- Basic and Optional Life Insurance
- Some Supplemental Plans (STD, Hospital)
11Changes for 2007 Plan Year
- In April 2007, employer contribution rates will
increase from 346.16 to 377.86/mo. for
individual coverage and from 715.92 to
787.60/mo. for family coverage. - Full-time employees will not be impacted, however
part-time employees will see a proportional
increase based on FTE. - Employees with 2 AFLAC Cancer plans will be
notified by the company and moved into one plan - People First will have system edits to preclude
future enrollments for eligibility or continuing
coverage restricted by age or existence of
overlapping coverage under the same vendor.

- AvMed is expanding service to Lake County
- United Health Care HMO is also expanding coverage
to Baker, Jefferson, Madison, Taylor, Wakulla
and Walton counties - Vista Health Plan HMO is offering a health
investor/HSA option in addition to their
traditional HMO plan -
13Changes for 2007UF Domestic Partner Plan
- UFs Domestic Partner plan will offer a Partner
Program providing health coverage at no cost if
both partners are active UF employees. - Due to IRS regulations, imputed income must be
assessed. The impact will affect the primary
partner (individual who has been employed
longer).
14Changes for 2007UF Sponsored - Long Term
Disability
- Minor changes for 2007 include
- Maximum monthly benefit will increase from 7000
to 9000. - Cost of living adjustment will change from 3 to
2 annually. - Definition of disability to reflect a 2 year own
occupation rather than a 3 year. - Social Security offset will now include any
monies an employee or their family receives as a
result of an employees disability. - Slight rate increase 90 Day Option -- .61/100
of salary coverage, and the 30 Day Option --
.94/100 of salary coverage. - This years enrollment is guarantee issue, which
does not require evidence of insurability.

- Changes to state sponsored benefits may be made
online via the People First website at
https//peoplefirst.myflorida.com or via IVR by
calling 1-866-663-4735 - Changes to UF sponsored benefits (Term Life, Long
Term Disability, Whole Life) may be made by
completing appropriate forms available on the HR
website at www.hr.ufl.edu and returning the forms
to the nearest HR office on campus

- Hosted kickoff events in fall 2005 with over
1,100 attendees. - Recommended making the no smoking within 50 feet
of an entranceway policy universal on campus.
The policy passed in fall 2005. - Conducted a Health at UF opinion survey in

how to make UF a healthier place to live, study

- Major institutional changes at UF in the past few

health behaviors - Mismanaged stress negatively impacts
interpersonal relationships both at work and at

health and quality of life

- What?
- Gators on the Go is a free web-based exercise
program that allows UF faculty, staff and
students of any fitness level to walk, swim,
bike, dance, run or even garden their way to a US
or European city of their choice. - Participants choose a goal distance and then log
their mileage from different activities as they
travel to their destination.

- The program begins on September 20, 2006 and runs
until December 6, 2006. - Participants will have the option to continue
logging their activity for the next year!

- What is it?
- It is a 401 (a) retirement plan for sick and
vacation payments at the time of separation of
salaried (non-OPS) employees. - It provides the maximum tax advantages for these
payments for employees and the University.

- BENCOR, Inc. is the plan administrator for the
University of Florida. U.S. Bank is the trustee
and handles all deposits into and distributions
of the plan for the University and BENCOR.

- Faculty, TEAMS, and USPS employees with payments
of 2,000 or more from sick and vacation
payments. Employees entering DROP after
implementation will also participate.

66Unemployment Compensation
- Eligibility Employees who lose their jobs
through no fault of their own or have their work
hours reduced against their wishes may be
eligible for UC benefits. - Who pays for UC benefits at UF? UF is classified
as a Reimbursable Employer, which means that the
states Department of Revenue (DOR) pays UFs UC
charges as they occur. DOR then bills UF
quarterly for those charges. The UF department
or budget entity that last employed an eligible
individual is responsible for all liabilities
incurred by UF. -
67Unemployment Compensation
- How much money will an individual receive each
week while on unemployment compensation? If an
individual is found to be eligible, the amount of
the UC check will be between 32 and 275 per
week, based on the claimants previous earnings.
Work history determines the duration of benefit
eligibility. Maximum period benefits may be paid
is 26 weeks. - How are UC benefits determined? The current
total amount of available benefits can range from
a minimum of 850 to a maximum of 7,150. The
benefit amount is basically equal to 1/26th of
1/4th of the base period gross earnings. The
maximum weekly benefits of 275 is equal to
1/26th of 7,150..

- Accident - means only an unexpected or unusual
event or result that happens suddenly. - Disability or death due to the accidental
acceleration or aggravation of a venereal disease
or of a disease due to the habitual use of
alcohol or controlled substances or narcotic
drugs, or a disease that manifests itself in the
fear of or dislike for an individual because of
the individual's race, color, religion, sex,
national origin, age, or handicap is not an
injury by accident arising out of the employment.
Subject to s. 440.15(5), if a preexisting disease
or anomaly is accelerated or aggravated by an
accident arising out of and in the course of
employment, only acceleration of death or
acceleration or aggravation of the preexisting
condition reasonably attributable to the accident
is compensable, with respect to any compensation
otherwise payable under this chapter.
71Workers Compensation
- "Injury" means personal injury or death by
accident arising out of and in the course of
employment, and such diseases or infection as
naturally or unavoidably result from such injury.
Damage to dentures, eyeglasses, prosthetic
devices, and artificial limbs may be included in
this definition only when the damage is shown to
be part of, or in conjunction with, an accident.
This damage must specifically occur as the result
of an accident in the normal course of
employment. - An injury or disease caused by exposure to a
toxic substance, including, but not limited to,
fungus or mold, is not an injury by accident
arising out of the employment unless there is
clear and convincing evidence establishing that
exposure to the specific substance involved, at
the levels to which the employee was exposed, can
cause the injury or disease sustained by the
employee.

- Arising out of and in the course of employment
- Arising out of pertains to occupational
causation. An accidental injury or death arises
out of employment if work performed in the course
and scope of employment is the major contributing
cause of the injury or death. - In the course of employment means that a worker
must have sustained an injury resulting from an
accident while engaged in employment, e.g.,
during working hours, on the employers premises
or at a place where an employee may reasonably
be, during the performance of employment duties
or during an activity related to the performance
of employment duties.
