• Recent Discharge following acute asthma symptoms
• Severe COPD treated with nebs, inhalers
  diuretics
• Not fitting community matron criteria
• High risk readmission frequent AE attender

Review/Home visit

• History asthmatic with life-long poor control
  ,anxiety, arthritis, CVD
• Examination anxious, depressed , wheezy and SOB,
  poor QOL
• Patient Carer engagement
• Medication R/V inhalers and technique,
  anti-depressants, CVD drugs statins, dosette
  box
• Multi-disciplinary team cardiology referral, DN,
  GP

Patient Goals

• Reduce asthma symptoms
• Improve mobility, QOL and social life
• Reduce depression anxiety
• Involve carer in decision making

Follow up

• Improved QOL, social life, mobility and
  confidence
• Asthma depression symptoms controlled
• No readmission/emergency services
• This intervention was the best thing that could
  have happened for Mrs A
