COAG National Action Plan on Mental Health
2006-2011 (NAP)
  • DoHA, FAHCSIA DHF are continuing to implement a
    range of the initiatives as outlined in the NAP
    in NT 
  • Some of the initiatives are being rolled out by
    other Australian Government Departments like
    DEEWR and DEET.
  • The corner stone of the Action plan which was
    signed of by all government Ministers is care
    coordination.
  • Each State and Territory has to report annual on
    there progress of the NAP

Care Coordination
  • A flagship of the NAP is providing a more
    seamless set of health and community services to
    people with a severe mental illness and /or
    complex needs who are at risk of falling through
    the gaps in the system.

. National Care Coordination Principles
  • Systematic arrangements for assessing the health
    and social needs of people accepted into the
    agreed target group eligible for care
    coordination
  • Development of a recovery/care plan which
    identifies the health and social care required
    from the range of providers
  • Appointment of an identified provider to keep in
    close touch with the service user and to monitor
    and co-ordinate care and
  • Regular review and, where necessary, agreed
    changes to the recovery/care plan
