- You will share this with your IEP team during
your yearly conference - Your team may include
- Your parents, guardians, and/or other adults
- IEP manager
- Transition/Work Experience Teacher
- Anyone else you and your IEP manager (special
education teacher) feel will be important - When will I use this transition planner?
- Beginning in 8th grade you will yearly update
the planner to keep the information current - By the time you are ready to leave high school,
you should have a summary of - Your disability, gifts and the supports you have
used - References and community resources available
- Classes and activities you have been involved
with - Vocational interest information and a job history
- Your research and future goals for independent
living, education/training and employment.

- Where Should I Keep This Transition Planner?
- Keep a copy of this planner on disc with your IEP
manager - When you change schools, the planner disc should
be sent to your new IEP manager - When you are ready to leave high school, the
planner should be printed and taken home with you
and used when needed - What Should I Keep With This Transition Planner?
- Keep copies of all information that will help in
planning for your future - Make copies of things from your school files such
as - Your most recent testing information
- IEP and credit checks
- You may also want to include
- A resume
- Letters of recommendation
- Agency reports
- Anything else you feel is important
