  • Referral scheme established 1996
  • Partnership PCT Link 4 Life
  • Over 800 new clients a year
  • Clients referred from health professionals
  • Scheme last initially 12 weeks and continues for
    another 6 Months
  • Various forms of Physical Activities
  • Reduced Cost

  • Improved muscular strength and mobility
  • Benefits breathing and circulation
  • Decrease risk of cardiovascular disease/cancer
  • Reduced risk of falls and accidents
  • Improves mental Health reduces depression
  • Increase independence and social interaction.

Referral Process
  • Client referred by health professional
  • Clients ring to book initial consultation
  • Initial consultation held at leisure centre
    Heywood, Middleton, Rochdale and Littleborough
  • Initial consultation take 1hour
  • Lifestyle questionnaire completed
  • Clients agree activity program
  • Blood pressure/Heart rate monitored
  • 12 week Referral card issued

  • Indoor Tai chi, pilates, yoga, circuit
    lightly, Gym
  • Outdoor Walking groups, green gym
  • Water based Swimming lessons, swimming,
  • water aerobics, deep water aerobics
  • Special Groups Ladies only, Prime time, Cardiac
    class
  • Instructor/staff Fully Qualified
  • Cost Free subsidised

Initial period
  • Clients receive a 2 week call
  • At 10 weeks clients receive an invitation to book
    2nd appointment
  • 12 week consultation takes 30 minutes
  • monitor progress
  • Future goals
  • Recheck Blood Pressure and Heart rate
  • Issue complementary Passport to leisure
  • Subsidised exercise continues for 6 months
