AIM STATEMENT All new patients admitted to 2
East rehabilitation will have their urinary
continence assessed with the care plan reflecting
their goals by November 2005

HOW THEY WERE IMPLEMENTED
- BACKGROUND
- Patient satisfaction survey 2002
- Identified continence as a priority for our
patients - Continence subcommittee established as a result
- Rehab (2 East) was selected as pilot unit for the
IC5 project - Research has shown that choice of discharge
location is largely affected by UI status - Staff on 2 East had participated in previous
continence care education - Strong support/buy in from multidisciplinary team
- PT and OT involved in promoting functional
continence (transfer and mobility training,
providing equipment, and ADL retraining) - Dietitians and Speech Pathologist involved in
ensuring adequate hydration - Speech Pathologist involved in enhancing
communication around toileting
- NEXT STEPS
- Validate existing hydration tool
- Evaluate current catheter practice
- Recruit additional resource people to ensure
spread and sustainability in promoting continence - Increase dedicated Nurse Continence Advisor hours
- Include continence care and promotion education
in orientation of all new staff
- Specific continence goals identified on
multidisciplinary care plan - Information from continence assessment form was
transferred to the bedside care plan - Nurses encouraged to use completed assessment
form as a tool to plan and carry out
interventions

WAS THERE A NEED TO CHANGE PRACTICE?
YES! Former comments from caregivers You
dont need to rush to the bathroom, you have a
brief on. This is normal, youre getting
older, wear a pad. Patient Comments I cant
go home if Im wet all the time. being able to
go to the bathroom is very important to
me. DID WE HELP? YES! Patients
Comments Oh yes, its much better using the
bathroom.
- Identified active continence goals on the
multidisciplinary care plan - Emphasized hydration and mobility goals and
toileting plans on the bedside care plan

2. Implemented simple cueing system to alert
staff to toileting plans for specific patients
