  • Child immunization registry.

  • Enhanced Care Management Medicaid social case
    management (Sedgwick County Medical Society).
  • Emergency Room case management in Wichita.
    (hospitals and partners).

  • Train TANEF/community college program
    participants about healthcare, disease prevention
    how to use insurance.
  • Fund school nurses through health departments.
  • Offer social- or disease-case-management to
    patients identified by Medical Management
    Information System (MMIS) data analysis as
    outliers based on disease, expenditure,
    emergency room utilization, or other means.

  • Use pilot projects to let the best
    entrepreneurial ideas lead the rest.
  • Engage many at the same time.
  • Plan for and listen to evaluators.
  • Demand accountability.
  • Health departments are already stretched!
    Resources needed.
  • Leverage federal funds using local expenditure
    and projects.
