Custom Page Templates

1.) This is the page template used for all Client
documentation. The red border, the location of
the logos, and the page numeration are static for
all document types. 2.)The components of the
page include Custom Logo for StayStat, edited
for print and web, inserted into PowerPoint, and
assigned an X and Y location. 3.) Figure 1 is an
example of a typical StayStat login page. All
applications created for this client had the same
color scheme. Every Client has their own unique
color scheme and specific instructional text. 4.)
The following pages are Draft examples of Help
Manual pages for the FirstLab Client. All images
were captured and pasted into Photoshop, edited
to fit on page. Exported as .JPG, then opened
in PowerPoint.

Participant User Account Request.
1.) Enter the Request User Account
information 2.) Enter First Name, Last Name 3.)
Enter the SSN without any dashes xxxxxxxxx 4.)
Enter the IVR PIN. 2.) When the Submit
button is clicked, the Participant is taken to
the next step in the Create User Account
process.

Participant User Account Request.
1.) In Figure 1, the Participant must enter the
Create Participant Login information. 2.)
Choose a Login ID that is at least five
characters long. 3.) Enter and Re-enter a
Strong Password that is at least 8 characters
long, that include alpha and numeric characters.
4.) Enter an email address. Once the User ID has
been generated, an email will be sent to the PHM
account manager and to the Participant. 5.) When
the information is complete click the Submit
button. 6.) Figure 2 is the Retrieve
Password Confirmation page. 7.) Click close and
you will be taken back to the login page.

Logging into PHM Online Participant

the Login Button. 3.) If the
Participants forgets the password or ID, click on
the Click Here link.

Overview of the Home Page
1.) Once the Participant enters the application
they will see their personalized Home page.
Across the top of the screen is a menu bar that
allows the participant to navigate to different
areas in the application. The menu is displayed
on all pages in the application. 2.) Along with
the menu bar, the printer icon is displayed
on each page allowing the Participant to print
any page in the application. 3.) The home page
displays the Participant testing information. The
figure below is an example of not being selected
for testing. 4.) Once the messages are read, the
Participant is required to click on the
Acknowledge button
This allows the system to record that the
Participant has logged into the application as
specified by program requirements.

Overview of the Home Page
1.) Figure 1 is an example of the screen that the
Participant will see when they have been selected
for test. 2.) This sentence is a list of the
selected tests/panels for the day. Other messages
of importance such as an expired credit card and
account status are also displayed on the Home
page. 3.) Once the messages are read, the
Participant is required to click on the
Acknowledge button
This allows the system to record that the
Participant has logged into the application as
specified by program requirements.

Important Functions
1.) An important part of the PHM Participant site
is the feedback option. The Feedback icon is
located on any page in the application. If you
would like to ask the PHM team a question, make a
comment, or report and error, click on the
Feedback icon. 2.) A pop up page will open. 3.)
Fill out the information and click the Submit
button. An email will be generated and
sent to the PHM team.

Participant Profile
1.) Click on the My Profile link on the menu
bar. 2.) Here you can view and edit Participant
information, like 3.) The Demographics function
displays the Participant information

1.) Click on the My Profile link on the menu
bar. 2.) Select the edit icon next to
Demographics. Information such as address, name,
and profession can be edited. 3.) Figure 2
displays the Demographic information that can be
edited. Note To edit any additional information
that is not available for modifying the
Participant must contact PHM to request the
changes. 4.) By selecting the edit icon next
to the Login Details, the Participant can update
their password. (Figure 3) 5.) Click the Submit
button or the Cancel button.

Editing My Profile functions
1.) Enter address information. Review for
accuracy. 2.) Click the Submit
button.

Billing Statement
1.) Click the Billing Statement link on your menu
bar. 2.) The Billing Statement module reports all
payment activity from 2004 to the present date.
Please note that the current months details will
not be present. 3.) Dates of tests for each month
are displayed in month and day format.

Forms and Reports
1.) Click the Forms/Reports link on the menu
bar. 2.) The No Call Report lists days the
Participant did not call in and the days that the
Participant did not call in and was selected for
testing. 3) The Test History Report lists the
details of the Participants tests. 4) The
Organization Holidays Report lists the
Participants Organization Holiday details. 5)
The Request ID Card Form allows the Participant
to request and pay for new ID cards. 6) The
Consent Form allows the Participant to give
their consent to release personal information. 7)
The Vacation Form allows the Participant to
inform PHM details of a schedule vacation.

corner. 2.) Fill in the requested information.
3.) Fax/Mail to FirstLab.

Participant Vacation Form
1.) Enter the Dates that you will be out of town.
2.) Enter Location City, State, Zip, and
Country. 3.) Select Reason for being out of
town. 4.) If your Reason is not in the list,
enter it in the Other text box. 5.) Insert
additional comments in the free text box. 6.)
Click Cancel to return to Forms/reports. 7.)
Click Submit when completed. 8.) Email/Fax
itinerary to PHM.9.) When Submit is clicked, an
email will be sent to PHM and the Participant.
