Whats the SmartCard Program?

• The SmartCard Program rewards you for working
  safely. How? When you work safely
• Youll earn points redeemable for gifts and
  prizes!
• Youll receive chances at winning a Dream
  Vacation!

How Does It Work?

• Every month, see your Supervisor to receive a
  SmartCard.
• Review the safety lesson with your crew members
  and answer the two T/F questions.
• Fill out your SmartCard with the information
  requested.

Record Your Answers

• To record your answers, call the SmartCard
  Hotline at 800-304-7783.
• Youll be prompted to key in your Hallett
  Employee ID , Supervisor ID , Crew MSHA , the
  Quiz printed on the SmartCard and your home
  phone .
• You also may record your SmartCard answers at
  www.incentivesonline.com/smartcard.

Dont call more than once to record your answers!

• Calling the SmartCard Hotline more than once a
  month to record your answers will disqualify you
  for the month!
• You may call the Hotline any time to check your
  point balance.
• Select the point query option when prompted.

You could be an instant winner!

• Every month, a random number of Hotline callers
  will hear a message that they are an instant
  winner.
• You can use your instant winner points
  immediately by ordering a gift from the Award of
  Excellence Catalog or save them up for a
  higher-level gift. The points will be recorded
  in a paperless account for you.
• Even if you are not an instant winner, you will
  earn a chance at a Dream Vacation just for
  calling.

All available gifts are shown in the Award of
Excellence Catalog, available from your
Supervisor. Gifts ship directly to your home.

How can you earn Scratchoffs?

• Every month if our entire company has zero lost
  time injuries, you will earn 1 Scratchoff
  SmartCard. If we go TWO months, you earn TWO
  Scratchoffs, and so on to a max of 12 per person
  per month. You receive your Scratchoff only when
  you complete your SmartCard questions for the
  month.
• Every month that your Supervisors Crew has no
  RECORDABLE injuries, you will earn 1 Scratchoff
  SmartCard.

How else can you earn Scratchoffs?

• If your crew passes its MSHA inspection with zero
  citations, you earn another Scratchoff.
• Each time you turn in a CLOSE CALL form you will
  receive 1 Scratchoff.
• Managers will be looking for you to demonstrate
  safe behaviors. If you are observed doing this
  youll earn a You Did It Right! Card. Call the
  number on this card to win additional chances at
  trips and prizes.

Your Scratchoff could be an instant winner!
Any Scratchoff could be an Instant Winner of
points that you can redeem for gifts from the
Award of Excellence Catalog. Redeem the points
immediately or save them up for a higher-level
gift. If youre not an instant winner, give your
Scratchoff to you supervisor, to be entered in
the drawing for one of the Dream Vacations.

Treat Scratchoff cards like cash!
Lost or stolen Scratchoff cards cannot be
replaced! Be sure to include them when mailing
in an order form for your gift!

Yes, you may combine Scratchoff card points and
Paperless points!
If you have earned paperless points as a
instant winner on the Hotline or from a You Did
it Right! card and you also have Scratchoff card
points, you may combine them when ordering a gift
from the Award of Excellence catalog. Include
your Scratchoff card when mailing in your gift
order form. Your paperless points will be
deducted automatically when your order is filled!

IMAGINE YOU are HERE!

• Dream Vacations will be awarded as follows

• One trip will be awarded every four months to an
  employee who has called every month into the
  SmartCard Training/You Did It Right! Hotline.
• At year end, the Supervisor who has the highest
  participation in the Training Program and You Did
  It Right! Program will earn a trip. The winner
  will be announced in November 2007.
• Every four months, all Scratchoff SmartCards that
  say Enter in Grand Prize Drawing will be placed
  into a drawing and one employee will earn a trip.
  Trips will be drawn April 2007, June 2007 and
  October 2007.

You qualify to win a Dream Vacation if

• You are still employed at Hallett
• You have not won a trip previously
• You have not had any safety violations

If you win a trip, you MUST use it within 6
months or it will be awarded to another employee.

  to report will cause your team to lose all awards
  and may result in termination/disciplinary
  action.
