  • Chief Complaint When first noticed problem? How
    it has changed since first started? What was it
    like at its worst? What has worked and not worked
    to help problem?
  • Current functioning of child What he likes to
    play with? What they enjoy together.

    do, where they do it, schedules and routines,
    play activities and how child reacts in different
    settings
  • Developmental and pregnancy history what child
    was like when first born? Parents reaction to
