Issue on-hold

• There is a desire to know whether the phone is
  on-hold
• Extension draft defines on-hold by media
  directions
• But
• Not clear what on-hold means consensus seemed
  to be user availability for communications
• In that case, its not a dialog state at all!
  Doesnt belong here. Maybe pidf-phone.
