    high-quality care throughout their lifetime,
    including support to optimise the control of
    their blood glucose, blood pressure and other
    risk factors for developing complications of
    diabetes

  • North West London Diabetes Dietitians working
    group
  • Dietitians should provide a comprehensive
    consultation with patients, using current
    evidence based knowledge and to tailor this to
    individuals, in order to achieve optimal diabetes
    management

    and follow-up plans
  • Criteria for audit is compliance with the

  • We are not meeting the standards outlined

  • Perhaps not providing the best quality care to
    patients with diabetes

  • Not knowing how to assess readiness to change
  • Lack of space on record card
  • Lack of time during consultation
  • Gaps in knowledge around blood results,
    medications and home blood glucose monitoring
