Page 1 is the checklist of required forms that
need to be completed with the application
Follow the checklist and instructions to ensure a
smooth and timely application process
The application booklet includes all the state
specific required forms as well as the Outline of
Coverage the MUST be provided to the client at
the time of application

Page 2 are the producer instructions to help you
complete the Leading Edge long-term care
insurance application and any additional required
forms
3Leading Edge Application
The Leading Edge app allows for spouse and/or
partners to apply together on 1 application
Part I are the knock-out questions. If any
question (1-5) is Yes, do not submit the app
Part II is the personal information for each of
the applicants
4Medical History
Part III is where your client provides the name
and address of their Primary Care Physician
Also in this section is where the applicants
medical history is gathered
5Medical History Details
Space is provided for any additional details that
are necessary due to a yes response to
questions 5-9 in Part III
6Part IV is for entering your clients policy
design such as Benefit Amount, Benefit Period and
Inflation Coverage
Part V of the application asks questions
regarding marital status and family members to
determine how much of a discount your client
would be eligible for
7In Part VI you complete how and how often the
applicant is going to pay for the long-term care
insurance policy
If your client is using a credit card as payment,
the Credit Card Authorization form does NOT need
to be completed if the credit card information is
entered in this section of the application
The one exception is if a 3rd party is paying the
premium via bank draft or credit card, we do need
the proper Authorization Form completed with the
app
8Your clients insurance history is completed in
Part VII of the application
Part VIII is completed if there are any special
requests
Complete Part IX if your client wishes to have us
notify another person, (3rd) party, if they fail
to pay their long-term care insurance premium
9Part X of the application should be read by your
client (and their spouse/partner if both are
applying) and as long as they understand and
agree with the statements, sign and date at the
bottom of the page
The application must be received in the Home
Office within 30 days of the date it was signed
The state version of the application that is
submitted must be the same as the state where the
application was signed
10The Producer/Agents Statement is Part XI and is
where you indicate which rates were quoted to the
client and if this policy is replacing another
LTCI policy
Part XII is the section of the app for your
information so that we can pay your commission.
You have the option to split commissions with as
many as 3 other producers by adding them in this
section
11You are required to complete the Advanced Payment
Receipt indicating the amount of money received
with the application. If credit card is being
used, indicate 0 for the amount received and
still submit this form. One copy is sent to John
Hancock while the other copy is given to the
client
The advance payment must be equal to a minimum of
1 month of the quoted LTCI premium
12Before John Hancock can begin the underwriting
process your client must read, understand and
agree with the statements on the HIPAA Medical
Authorization
This form allows our underwriters to obtain the
applicants medical records, if necessary, so
that we can make the decision to offer or decline
the application for coverage
13If this Leading Edge policy will be replacing an
existing long-term care insurance policy, the
Replacement Notice must be completed. One copy
is sent to John Hancock while the other copy is
given to the client

The application booklet also includes the
Long-Term Care Insurance Personal Worksheet
This 3 page worksheet has questions to help
determine suitability and therefore must be
completed by each applicant and submitted along
with the long-term care insurance application
