DATABASE RESPONSIBILITIES
Enter all SY 05-06 YES codes (YES) by 6/23/06.
Enter SY 05-06 final grades and GPAs by 6/23/06.
Enter SY 05-06 spring AIMS results by 6/27/06.
Enter summer 2006 AIMS results .
Web-scan all services / student parent
participation, including honors course
enrollment, according to back up documentation.
Maintain records, back up documentation
(including sign-in sheets) to evidence all data
entered.

REPORTING REQUIREMENTS
Submit 3rd quarter performance report by 6/29/06.
Submit 4th quarter performance report by 9/12/06.
Submit Professional Development Report 2 for
Period 3/1/06-9/10/06 by 9/11/06.
Between August 15, 2006 and September 10, 2006
contact every GEAR UP student who graduated in
May/June report their status to State Office on
GEAR UP Student Post Secondary Status Report
Form, due 9/14/2006.

FINANCIAL RESPONSIBILITIES
Gather and report value of in-kind or cash
contributions for all donated services / time,
money, and goods, including non-federal
scholarships.
Submit financial reports. (Documentation of
non-federal scholarships reported as cost share
must be attached to submitted reports.) 6/30/06

Ensure Counseling Department has ACF phone number
for GEAR UP Scholarship questions and referrals.
