  • Enrolled in Medicare Part B and entitled to Part
    A?
  • Permanent resident in service area?
  • Do you have End Stage Renal Disease (ESRD)?
  • Federal law will not allow us to accept anyone
    who has end stage renal disease (kidney failure),

    offered by Humana within the same state, or

    still a part of the Medicare program. With all

    Medicare approved Prescription Drug Plan
