Open Enrollment 2009

• Open Enrollment Benefit Fairs
• To be conducted from Nov.2 Nov. 30
• Letters sent requesting time off for employees
• 34 benefit fairs scheduled
• Each benefit fair is scheduled for about 1 ½
  hours
• Begins with PowerPoint presentations followed by
  QA with carrier reps
• PowerPoint presentations will be available on
  website at www.hieutf.org

Open Enrollment 2009

• Open Enrollment packets to be sent directly to
  Department Personnel Officers
• Packets contain Open Enrollment Notice which
  lists the plans that employees are enrolled in,
  and everyone else that is enrolled on the plans.
• A blank EC-1 is included in the back of the
  Reference Guide. Employees will use this to
  make all Open Enrollment changes.

Open Enrollment 2009

• Employees who do NOT receive a Reference Guide
  for actives, may download it from the EUTF
  website at http//eutf.hawaii.gov
• Premium contribution tables also will be posted
  on the website.

Open Enrollment 2009

• Form EC-1 to be submitted to your personnel
  office.
• Those NOT making any changes, keep enrollment
  notice for their records.
• If changes desired, form EC-1 form must be
  completed and signed by employee.
• Must be certified by personnel before routing to
  EUTF
• The deadline for employees to submit the EC-1
  form is
• November 30, 2009
• All changes will be effective January 1, 2010

  employees who have made a change to their plan
  enrollment.
• Those NOT making any changes, keep their Open
  Enrollment Notice as their confirmation.
