homebound?

- Principal
- Special Education Coordinator
- Students Physician
4What are the steps to place a student on
homebound?
- A Homebound Instruction Application will be
completed by the students principal, and/or
special education coordinator, and/or physician.
Applications are available on the Sedalia School
District website, www.sedalia200.com, documents
and forms, pupil services, homebound icon. - The application will be turned into Central
Office, attention Linda Myers. Instruction will
not begin until the homebound application has
been submitted so applications must be submitted
on a timely basis. - When the application is received at Central
Office it will be reviewed by Julie Dill,
Director of Homebound Services. - The students guidance counselor, academy level
counselor at high school, locates a homebound
teacher making sure that no one teacher has more
than 2 students at a time. A homebound teacher
list is available from Linda Myers, Central
Office. Homebound teachers must be certified
teachers /substitutes of the Sedalia School
District - A homebound teacher approval form will be
immediately sent to the homebound teacher along
with homebound payroll forms and a copy of the
homebound parent letter.
5Completing the Homebound Application Form
- Guidance Counselors/Special Education Coordinator
will complete all information in sections I
student information and II school district
information. - Students Special Services Coordinator will
complete section III, educational information.
(if special education student) - Students Physician will complete section IV,
medical information (if necessary). - If a student requires services past 9 weeks
(unless original application was extended by a
physician), please submit a new application with
the extra weeks required (up to an additional 9
weeks). If the request is being extended by a
physician, a note stating the expected date of
release will be sufficient.
6Homebound Instruction Application This form
will be completed by the necessary school
administration and sent to central
office. Copies of this form are also available
on the website
7How do I submit my homebound payroll?
- Homebound students are seen after school, during
a teachers flex time, or after regular school
hours by a teacher/substitute who is on the
homebound payroll. - Rate of pay is 15.00 per hour regardless of the
number of students seen during that hour. - Complete a Homebound Payroll form for each
student seen during the month. - Obtain a signature from a parent, library staff,
or other witness, not the student, that was
present during the instruction daily. - If a student fails to keep an appointment twice
in a row it is the responsibility of the teacher
to contact that students guidance counselor.
Teachers will only be paid for 2 no show
appointments per month. - Mileage is only applicable for out of town visits
and requires prior approval. - Homebound teacher will turn original payroll form
(keep a copy for their file) into their students
guidance counselor by the 2nd of each month. - Guidance counselor will compare payroll form to
attendance on SISK12. (If there are
discrepancies the guidance counselor reviews
dates with attendance secretary making sure
payroll form and SISK12 match.) - Guidance counselor sends payroll form to central
office, fax 826-1020. Forms must be on my desk
on the 3rd of each month. - Central office will update Homebound reports and
forward to payroll. Central office will check
homebound reports making sure a payroll form was
turned in for active homebound students. If a
payroll form has not been received, central
office will contact the students guidance
counselor who will follow-up with the teacher.
8Homebound Teacher Approval Form This form
will be sent to the homebound teacher after the
homebound application has been approved by the
homebound director.
9Homebound Payroll Form The homebound teacher
will receive the homebound payroll form along
with the teacher approval form. Please make
copies as needed. Copies of this form are also
available on the website
10How do I arrange an instruction time with the
student?
- The Parent Information Letter will be mailed to
the students home. - This letter lets the parent know that a homebound
teacher will be contacting them to arrange a
convenient time and location to meet with their
student regarding homebound instruction. - During this instruction time the teacher will
provide the necessary assignments and work with
the student 5 hours per week (unless otherwise
specified) regarding those assignments. (i.e.
one hour per day or hours totaling 5 hours during
a week.) - Following every instruction session the parent or
an adult that has witnessed the instruction will
be asked to sign, on a daily basis, a teacher
payroll form. This form will verify that
instruction was provided to the student.
11Homebound Parent Information Form The parent
information form will be sent to the homebound
students parent the same day the teacher
approval form is sent to the homebound teacher.
