Who is Eligible

• Individuals
• Organizations
• Civic Services
• Community Services
• If the nominee is a member of the Federal
  Service, the individual must be an active member
  of AFOS and AOA

Member Definition

• An active AFOS and AOA member is a member who
  is in good standing. This means the individual
  has already applied for membership to AFOS and
  AOA and dues are paid in full prior to the time
  of submission of the nomination to the awards
  committee.

Who may Nominate

• Any active AFOS member

Nomination

• All accomplishments including but not limited to
• military career
• involvement in AFOS/AOA
• other organizational involvement
• service to optometry
• service to political subdivisions
• service to the public

• AOA Award Guidelines for
• Distinguished Service Award

Submission Process

• Deadline November 15
• E-mail submissions to Dr. Carol Rymer

Selection Process

• The Judging panel
• AFOS executive council
• Maximum 3 recipients per year

Notification

• Recipients and nominees will be notified the
  first week in January to allow for planning in
  order to attend the Annual Federal Service
  Optometry Meeting

Presentation of Award

• AFOS Annual Meeting
• Held in conjunction with Southern Educational
  Congress in Optometry (SECO)
• Atlanta, Georgia
• February following nomination deadline
