History

• Started 1 August 2001 as PMS Pilot-Greenfield
  site
• 2 WTE GPs 1 Nurse Practitioner for 2000 list
  size
• Currently 4800 patients
• Became LLP in 2004
• Partners-1 GP, 1 Nurse Practitioner, 1 Business
  Manager

Why did it start?

• Northampton GP identified some people not
  accessing primary care
• Convinced local Health Authority to find some
  funding
• Built evidence base
• PCG agreed to feasibility study
• Supported PMS bid

Who is the service for?

• Homeless
• Travellers
• Asylum Seekers
• Refugees
• Anyone who finds it hard to access primary care
• In fact anyone who wants to register in our
  catchment area

Whats in the service

• Now 2.6 WTE GPs
• RCGP Part 1 2 qualified
• Substitute prescribing for 200 patients
• 2 x Mental Health nurses employed by practice
• 120 patients on SMI register
• Plus all the GMS services avaliable in any
  practice
