Commitments/Agreements

• VL
• -UC you have to request via NAP program by
  yourself within 1 wk after sent sample (together
  with request form that fill in complete data
  especially HN and NAP number and then VL results
  will be reported within 1-2 wk via NAP program
  and official letter, respectively
• -nonUC results will be reported within 1-2 wk
  via NAP, e-mail and official letter, respectively
• You have to pay once a month and you
  will be included in blacklist and defer for
  performing VL if you could not pay within 3
  months after sent sample

• -nonUC If perform at DPC7, will be charged
  10,000 baht/test and reported via e-mail and
  official letter within 1 month
• -UC Patient who showed VLgt2000 c/ml, request

  then reported via NAP program and official letter
  except resistant case, will be reported by fax
  prior to by official letter
