• encourage and support these teams to RECORD, for
  individual patients, where guidance NOT used, and
  WHY
• encourage and support teams to review and discuss
  this data (maybe producing local versions of the
  guidance)
• make suitable financial and organisational
  arrangements
• monitor all this at Board level
