3- What is osteoarthritis?

- Osteoarthritis, also known as degenerative
arthritis, hyperplasia osteoarthritis or aging
osteoarthritis, is a kind of degenerative change
due to softened, disintegrated or locally
denudated cartilage matrix of joint as well as
insecondary hyperosteogeny at the joint edge.
4- It is characterized by joint pain and
dysfunction, including joint malformation.
Generally, diseases, including spur,
hyporosteogeny, bowleg, cervical spondylosis,
rheumatism, rheumatoid arthritis, for example,
slipped disc caused by degenerative change all
fall into the category of osteoarthritis.
5- Status of Osteoarthritis

- Osteoarthritis is neither so serious as
cancer and AIDS nor so recognized as heart
disease and diabetes, but it troubles people,
racks patients, and ranks 1st among chronic
diseases which disables adults. -
6- Status of Osteoarthritis
- Approximate 100,000,000 Chinese, which
accounts for 10 per cent of the national
population now suffer from osteoarthritis. Being
common among middle ages, elders and specific
occupations, including athlete and teacher and on
the rise among general population, it becomes the
major chronic disease which disables us and
affects our health and living quality.
7- As a chronic disease, osteoarthritis requires a
long period of treatment. Due to joint
dysfunction, patients are restricted for movement
and shall be replaced with artificial joints in
the late stage, which impairs patients living
and work ability greatly and brings heavy
financial burden to patients and their families.
According to statistics, by 1999, approximately
16,000,000 Americans out of the overall
population of 280,000,000 suffer from
osteoarthritis, the annual outpatients reach
80,000,000 person trips and 600,000 surgeries are
performed. The annual average treatment expense
is up to USD2,000 3,000 per capita.

and every 12th October is named the International
Arthritis Day.

- Man cant stand without bone that cant move
without joint. - The typical structure of movable joint is
comprised of articular surface, cartilage, cavity
and capsule.
Articular cartilage
Where articular cartilage is the most important
component.
11- 1. Normal Articular Cartilage
Articular cartilage is a layer of smooth and
elastic tissue with 2-7mm of thickness covering
on articular surface. Working as a buffer with
decent elasticity or a perfect cushion between
joints, it bears various high-loading pressures
and impacts in cycle in our life. Healthy
articular cartilage guarantees the free motion of
the joint as well as buffers and protects the
joint by absorbing vibration energy resulting
from motion.
12- 2. Abnormal Articular Cartilage
With age, the articular cartilage of our body is
worn, lost and degenerated inevitably. As being
used and consumed over time, much more articular
cartilage is lost, but the self restoring and
rebuilding capacity is declining. In this case, a
part of cartilage becomes softened, eroded,
thinner and even disarticulated to produce a
tough surface and hyperosteogeny at the edge. For
lack of protection from cartilage, pressure
between joints increases, pain, swelling,
stiffness, sound or friction is occurred to the
joint both in still or in motion, and we will
fail to move as free as we did before.

14- Clinical Manifestation
- Symptom
- It is mainly demonstrated by red, swollen,
heat, aching and dysfunction at the joint in
question, which becomes sensitive to change of
temperature and humidity in the climate. - Early stage patient feels slight discomfort
and annoyed. - Common patient feels stiff, unbearable ache,
and be tired for walking up and down stairs. - Critical patient the joint is deformed, and
the joint moving range is reduced.
15- Main Symptoms
- 1. Ache, Swelling
- Ache is generally occurred in motion
- and be exacerbated when walking up
- and down and being loaded. Patients
- are always waken by ache at night and
- kept awaken the night through. With
- ache, the joint motion is accompanied
- by sound or friction frequently.
16- 2. Restricted Motion
- It occurs slowly.
- In fact, a great number of middle ages and
elders feel stiff in the joint when getting up in
the morning, several minutes or longer time is
required for restoration. Such phenomenon is
called morning stiffness. - Patient feels stiff and ache in the joint
when standing up after a long time of sitting,
and is only allowed to move after minutes of
massage. So patients may fail to stand up at once
in emergency.
17- 3.Dysfunction
- With the growth of osteoarthritis, the joint
will become deformed or disabled for motion, and
finally lose all functions. Joints in critical
conditions shall be replaced with artificial
ones.

- Osteoarthritis may be occurred to joints at any
part of the body from head to foot. Typically, it
is concentrated in cervical spine, hand, lumbar
spine, knee and ankle joints.
20- X-ray film
- 1. narrow gap between joints

- 3. hardened lower cartilage and degenerated
capsule. - Clinical symptoms of osteoarthritis are not
necessary consistent with those illustrated by
the X-ray film. In practice, approximate 50 per
cent patients who suffering from osteoarthritis
carries no distinctive symptom.
21- Pathogenesis
- 1. The cartilage is degenerated and tenuated
with the age, but the self restoring capacity is
decreased. The incidence of osteoarthritis grows
as human and society become aged since aging
takes the most important contribution for
osteoarthritis. - 2. The joint fails to bear pressures from
the body, and bones conflict with each other
without buffer. Obese people due to excess
nutrients and unscientific dietary habits in
modern society are much likely to suffer from
Osteoarthritis. - 3. The incidence rate is high among special
occupants, including athlete, teacher, dancer,
miner, female textile worker because joints are
over used and damaged frequently. - 4. Osteoarthritis caused by infection, low
immunity or wound, etc.

- It is found by medical research that
glucosamine is the basis of articular cartilage.
It is the raw material for composing and
producing cartilage and fluid and stimulates the
generation of cartilage cell for new cartilage. - The cartilage starts to aging since 25. With
the growth of our age, glucosamine is reduced
from the body, which may directly result in

27- Status of treatment for bone joint diseases

including traction, massage, acupuncture,
infrared radiation and iontophoresis are applied
however these methods can only alleviate the
symptoms for a short while but fail to heal the

- a. Oral medicine traditionally, non-steroidal
anti-inflammatory and antalgesic drugs are used. - It relives pain by inhibiting the process in

drugs such as indomethacin may cause serious side

Such drug may damage liver and kidney function,
for example, causing acute renal failure and

which results in dizziness, headache and
neurological disorder or even causes necrosis of
the femoral head.

- Clinically, a lot of external medicines
including white and black salve mulls are used to
treat arthritis. Commonly used Longhu Bone
Strengthening Salve Mull and Musk Bone
Strengthening Salve Mull etc. relive pain and
improve local blood circulation effectively, but
they tend to damage the skin and fail to repair
cartilage fundamentally. Therefore, they cant be
named ideal treatment methods.
