LAF - Legalities
  • We are pushing forward on the WC forms
  • Each of your designated reps will continue to
    fill out the form
  • ACIG will continue to capture the information
    and can run reports accordingly

  • ON THE GENERAL LIABILITY CLAIMS
  • The ACIG Sr. Claim Representative or the
    Claims Consultant will complete the form once
    notice of the loss is obtained.
  • We believe this offers the legal work product
    protection.

  • Root Cause
  • The most basic cause(s) that can be reasonably
    identified and that management has control to
    fix.
  • This definition helps limit an investigation to
    finding correctable causes that lead to
    implementation of corrective actions
  • It helps answer the question of when is enough,
    enough?
  • If we can focus on fixable causes of an
    incident, that when corrected, will prevent a
    recurrence, then we succeed in true risk
    management.

Root Cause
  • Typically we start at the first known fact
  • he fell because he was not tied off
  • This is a human performance factor.
  • Or, we could have an equipment failure
  • the brakes failed on the dump truck
  • But these are considered the BASIC cause(s) and
    should just be the catalyst to drill down further.

  • Lessons Learned
  • Thoughtful consideration should be given to the
    recommendations given in this section.
  • These should be specific, measurable,
    accountable, reasonable and timely.
  • It is not about fault finding, blame, or shame.
  • Language should be precise, factual and
    non-judgmental with instructions on corrective
    measures based upon the root cause determination.

  • We are putting together a Root Cause Training
    program
  • Training will be determined on a regional basis
  • Promoting Train the Trainer concept so field
    can be trained as well

  • Reports are there for the asking.
  • For those who use RM for the Contractor side, all
    fields are there for you to capture for your own
    information
  • We will continue the process
  • We believe that the information can be useful in
    areas of loss prevention, hiring and training
