  • Is responsible for co-ordinating safeguarding
    arrangements in Newcastle and for ensuring the
    effectiveness of arrangements
  • Provides strong leadership, scrutiny and is a
    driving force for effective safeguarding in
    Newcastle

How does LSCB work fit with the childrens trust,
and with the inspection framework?
Feeding into the wider childrens trust
arrangements. Ensuring that the Children and
Young Peoples Plan covers safeguarding
effectively. Ensuring that effective
multi-agency safeguarding training is delivered
as part of the local workforce strategy.

Serious Case Reviews
  • Set out in statutory guidance Working Together
    (2006)
  • Reviews must be carried out where a child has
    died or been seriously harmed and abuse or
    neglect are suspected
  • Should always consider a SCR where
  • A child sustains a potentially life threatening
    injury or serious permanent impairment of health
    or development or
  • Has been subjected to particularly serious sexual
    abuse or
  • A parent has been murdered and a homicide review
    is being initiated or
  • A child has been killed by a parent with a mental
    illness or
  • The case gives rise to concerns about
    inter-agency working

Purpose of a Serious Case Review
  • To establish whether there are lessons to be
    learned about local inter-agency working
  • To identify the lessons, how they will be acted
    upon and what will change
  • To improve inter-agency working to better
    safeguard children

Process
  • Ofsted notified of serious incident or death
  • NSCB standing SCR Committee - sets up Serious
    Case Review Panel to consider the circumstances
    of the case
  • Recommendation to the Chair of NSCB
  • IMR report authors identified must have
    appropriate skills and experience
  • Overview author commissioned
  • Parents family advised of process outcome

Reports
  • Single Agency Reports from all agencies involved
    with case
  • Close scrutiny by Committee to ensure open,
    frank, accurate information
  • Each agency makes its own recommendations with
    timescales
  • Chronology of involvement drawn up merging all
    agencies actions
  • Overview Report written, to bring all Single
    Agency Reports together, make Findings and
    further recommendations
  • Executive summary released to public

Action
  • Findings of Single Agency reports may have been
    acted on immediately or during reporting process
  • Overview Report stage is followed by an
    all-agency Action Plan with timescales for
    completion of all recommendations
  • Feedback, staff training, changes to practice,
    policy, if required
  • NSCB and Ofsted monitor progress
  • NSCB signs off completed Action Plan, closes SCR
    down
