must - Be warded in a hospital as an in-patient or for
day surgery. - Be quarantined under any written law.
2- Be certified by a medical practitioner who can
admit patients into an approved hospital,
including medical practitioners from national
specialty centers and ambulatory surgical
centers. - Once you have met the eligibility criteria, you
can apply for hospitalization leave by - Informing your employer as soon as possible of
your hospitalization. You should do this within
48 hours of admission, if possible. - Providing your employer with a medical
certificate (MC) from the doctor who admitted
you to the hospital. The MC should state the date
of admission, the date of discharge, and the
reason for hospitalization. - Completing and submitting a hospitalization leave
application form to your employer. This form is
usually available on your company's intranet or
HR portal. - Your employer may also require you to provide
additional documentation, such as - A copy of your hospital bill.
- A letter from your doctor confirming that you
were unable to work due to your hospitalization. - Once your employer has approved your
hospitalization leave application, you will be
