Requirements of License By Examination 1) Copy
of diploma or transcript from an approved
Osteopathic Medical School, showing date of
graduation 2) Original, official transcript of
NBOME or COMLEX scores if passed Level 3 within
the past 7 years if applicant has been in
continuous practice in another state for 7 years
and had previously passed exams, then exams do
not need to be retaken 3) Copy of certificate or
official letter showing completion of an
internship or first year of residency 4)
Verification of licensure and disciplinary
history from each State applicant was licensed
5) Application Fee - 400 Reciprocity Not
Available License Fees Initial - 15.90/mo
through the end of December (maximum of 190.80)
Renewal (Biennially on or before Feb. 2) - 636
Locum tenens registration available for 90-day
period upon approved application, including
documentation of current, unrestricted license in
another jurisdiction. May be renewed once for
additional 30-day period. Continuing Education
20 hours of approved each year with a minimum of
at least 12 hours of Category 1-A CME and not
