What is Form 1096? Form 1096 is a form used by
the Internal Revenue Service (IRS) to report
summary information for certain types of
information returns. The form is used to report
the total number of information returns filed, as
well as the total amount of money reported on
those returns. The form is required to be filed
by any person or entity who files 250 or more
information returns during a calendar year. This
includes any person or entity who files 1099,
1098, W-2G, or other similar forms. If you are
required to file Form 1096, you will need to
