Introduction
  • The term ovarian reserve refers to the number
    and quality of your eggs, also known as oocytes.
    If you have diminished ovarian reserve, this
    means that the number and quality of your eggs is
    lower than expected for your age.
  • Ovarian reserve refers to the reproductive
    potential left within a womans two ovaries based
    on number and quality of eggs. Diminished ovarian
    reserve is the loss of normal reproductive
    potential in the ovaries due to a lower count or
    quality of the remaining eggs. Ovarian reserve
    can also be considered a part of the biological
    clock, but this clock can vary from woman to
    woman. Some women continue to be fertile in their
    40s, while others begin to lose their fertility
    in their 20s. In general, women start losing
    ovarian reserve before they become infertile and
    prior to the end of their menstruation, according
    to the American Society for Reproductive Medicine.

Causes 
  • The main causes of diminished ovarian reserve
    are
  • Cigarette smoking.
  • Genetic abnormalities (Fragile X and other X
    chromosome abnormalities).
  • Aggressive treatments (radiation for cancer).
  • Ovarian surgery, such as for endometriosis.
  • Idiopathic (this means that there is no apparent
    cause).
  • prior ovarian surgery
  • chemotherapy
  • pelvic infection
  • autoimmune disorders

Symptoms 
  • There arent many noticeable symptoms of
    diminished ovarian reserve. However, those with
    the condition might experience any of the
    following symptoms
  • Difficulty getting pregnant
  • Late or absent menstrual periods
  • Shorter menstrual cycles than average, with the
    average being 28 days
  • Heavy menstrual flow
  • Miscarriage

MEASURES
  • Ovarian reserve tests include both biochemical
    tests and ultrasound imaging of the ovaries.
    Biochemical tests of ovarian reserve can be
    divided further into early-follicular-phase
    measurements of FSH, E2, or inhibin B
    measurement of cycle-day-independent
    antimullerian hormone (AMH) and provocative
    tests, such as the clomiphene citrate challenge
    test (CCCT). Biochemical measures of ovarian
    reserve are intended to directly or indirectly
    measure the oocyte or follicular pool.
  • Inhibin B and AMH are glycoprotein hormones
    produced by small ovarian follicles and are
    therefore direct measures of the follicular pool.
    Whereas AMH is primarily secreted by primary,
    preantral, and early antral follicles, inhibin B
    is secreted primarily by preantral follicles. As
    the number of ovarian follicles declines with
    age, both AMH and early-follicular-phase inhibin
    B concentrations decline.

Diagnosis 
  • If youre under 35 years of age, you should see a
    doctor if youve been trying to get to conceive
    for a year. If youre over 35, see a doctor after
    six months. If youre over 40 and youre trying
    to conceive, its best to talk to your doctor
    right away.
  • Diminished ovarian reserve is diagnosed through a
    few blood tests that measure follicle stimulating
    hormone (FSH) and anti-Mullerian hormone (AMH)
    levels. Both of these hormones play an important
    role in menstruation and reproduction.
  • Its natural for FSH levels to increase and AMH
    levels to drop as you age, so its important to
    assess these hormone levels against a baseline of
    your age. Those with diminished ovarian reserve
    will have higher FSH levels and lower AMH levels
    compared to others their age.
